who are entering Jockey Hollow School
Immunizations
The Monroe Board of Education requires proof of immunization and physical examination dated within the year prior to enrollment in order to be in compliance with Connecticut State Law.
We are required by state regulations to exclude students from school until all immunization requirements have been met.
Health Screenings
The State of Connecticut mandates that vision, hearing and postural screenings are conducted annually in accordance to Connecticut General Statute Sec. 10-214 and 10-206.
Vision screenings are done in kindergarten through sixth grade and again in ninth grade. Students in kindergarten through third grade should be able to pass the 20/30 line with each eye and fourth grade and above able to pass 20/20 with each eye.
Hearing screenings are done in kindergarten through third grade and again in fifth and eighth grades. Students should pass pure tone stimuli at the following frequencies and intensities:

Postural screening checks for scoliosis (a lateral curve of the spine) and kyphosis (a round curve in the upper spine when viewed from the side). Postural screenings are done in grades five through nine.
Anytime a student fails a screening, a few weeks later the school nurse will recheck the student. If the student should fail the screening for a second time, a referral letter will be sent home requesting further examination. Please return the referral letter to the school nurse after your child has been evaluated.

At Jockey Hollow hearing
screening will take place in
grade 8 and postural screening (scoliosis) will occur in
grades 6, 7 and 8.
Medications
In accordance with sections 10-212a-1 through 10-212a-7 of Connecticut’s Public Health Code, the school nurse is able to administer medication to students. Medication orders and permissions are only for the current school year. All orders and permissions need to be renewed each year. Medications cannot be stored over the summer; therefore any medication not picked up at the end of the school year will be disposed of.
Prescription Medications to be given at school REQUIRES:
Prescription medications may be given according to Connecticut State Law and Regulations which requires an MD, DDS, OD, APRN or PAC’s written order and parent/guardian’s signed authorization. Specifically:
1. A written and signed order form by a licensed prescriber.
2. A parent or guardian signature on the form.
3. An adult
MUST deliver medication to the clinic. Students are NOT permitted to carry medication to and from school.
4. Medication must be in its original container with the pharmacy label attached.
5. No more than a 45 day supply may be kept in the clinic.
Self Carry Medications:
A student may self administer medication (ie inhaler, Epipen, insulin) only with the written authorization of the prescriber and parent/guardian, the situation is deemed safe by the school nurse, the principal and appropriate staff are notified and the medication is transported and maintained under the student’s control.
